His physician had noted on the lead testing form that CR would have lead testing after he turned one year old. CR is now over two years old without any evidence of lead testing. Findings: Discussed regulation with Provider. Notified her of the need for a six foot fall zone for climbing equipment and educated her about entrapment issues. Provider needs to add vertical beams on the stairs of the playset which are less than 3 1/2 inches apart or enclose it completely. Entrapment can become an issue when there is a gap between 3 1/2 inches and 9 inches. Please alleviate the opening on the sides of the stairs and the openings between each step of the set of stairs (which are currently 4 inches apart). Please discuss ideas with Licensing Specialist before making changes to ensure compliance before modification.

They are different types of drills and should be conducted as such. Please write a letter stating how you will become compliant with this regulation in the future.
