| Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 3/25/2026 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Emergency Disaster Plan update form was completed at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation link to the Provider. The following was discussed but not limited to: 1. New rules. 2. Paperwork associated with the new rules. 3. Email addresses on children's Records. 4. Required postings in the gym (activity schedule, capacity, evacuation plan) 5. Annual training hours for staff. 6. CBC submittal requirements. 7. Staff records must be complete. 8. Paperwork for staff that must be completed prior to their starting date of employment. 9. Brooms must be inaccessible to enrolled children. 10. Posting the current license. 11. Ensure TB test results are maintained in staff files. 5. Ensure documentation of education is in each staff file. 6. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. **Please submit the copy of the fire approval, when obtained, for the March 2024 Paradise Valley fire inspection. The following was discussed, but not limited to: 1) Foreign degree verification process for education's equivalency for any staff who have documentation of foreign education. There were 5 staff files reviewed. 5 of the 5 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection. | |||
