| Initial Comments: The following deficiencies were observed at the time of the annual compliance inspection conducted on 8/27/2025, and are subject to changes pending programmatic review. Please submit the Plan of Corrections through the LMS Portal within 10 days of receipt of the Statement of Deficiencies. Five of five Fingerprint Clearance Cards were verified as valid via the DPS website. The following was discussed, but not limited to: 1) New rules effective 8/3/25, 2) Definition of walking, in regard to the one year old classroom, 3) TB test attestation section to be completed by facility, and 4) Medication requirements. | |||

| Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 9/3/2024, and are subject to changes pending programmatic review. Compliance Officer #1: Jennifer Flicker Compliance Officer #2: Chloe-James Rossi The Plan of Corrections is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed, but is not limited to: 1) Ensuring rosters are accurate if the licensee chooses to do both an electronic roster in addition to a paper roster, 2) Anniversary fee due in October 2024, and 3) Effective May 2024, DCS background clearance applications are now required to be submitted through the new DCS portal for all new staff and for any other staff if the staff did not obtain DCS clearance in the past. There were 6 staff files reviewed. 6 of the 6 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection. | |||
