Findings: After a review of 5 staff files, it was determined the following was lacking: Staff #2 (start date 8/11/2014) had a copy of a fingerprint clearance card with an expiration date of 6/11/2020. Also, the Surveyor checked the DPS website at the time of the inspection and verified the status as "expired." Staff #1 stated Staff #2 has worked in the facility when it was open for 4.5 weeks in June 2020, 3 weeks in July, 2020 and currently since 8/17/2020 when the facility reopened again. Staff #2 was observed working in the front office and infant room during the inspection.

Findings: In Room 10 (infants), the paper towel dispenser was not located in the approved diapering area, but in a divided off area on the far side from the diaper changing area. In Room 7 (2 yr. olds) and Room 6 (2 yr. olds), the paper towel dispensers in the diaper changing areas were observed empty. There was a stack of paper towels on the counter near each of the dispensers.

Findings: In Room 11 (infants), the following was observed in the diaper hand washing sink and/or around the diapering sink and changing surface: plastic bowls and toys. In Room 9 (1 yr. olds) and Room 6 (2 yr. olds), there was a clipboard with the diaper changing logs on it on the diapering counter.

I have used this provider for more than 6 months

I was very dissatisfied with substandard care regarding my infant's nutrition and injuries received at school. I tried to work with Chanen regarding these concerns but they failed to communicate with me. I had no choice but to switch schools. We love the new school and have had no similar concerns.
