Findings: Based on the Surveyor's observation, in the 4's - 5's Classroom, a child walked out of the door into the hallway and started walking towards the hallway restroom alone and unsupervised. The Surveyor stood in the hallway watching the child walk towards the restroom and informed a staff member that the child walked out of the classroom. A staff member then walked into the hallway and escorted the child to restroom.
