| Initial Comments: The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 5/14/2025, and are subject to changes pending programmatic review. A full inspection was conducted at this time. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. Please send the Department a copy of the Statement of Services. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Emergency Disaster Contact Form was completed at the time of the inspection. The Empower Survey was emailed at the time of the inspection. During the exit interview, the following was discussed, but not limited to: Playground cleaning & maintenance, Change of space utilization application process (Multi-purpose Room) and based on a walkthrough of the space, the following was discussed: Repair & equipment storage, required room postings, outlets, accessibility to non-licensed spaces. | |||

During the exit interview, the following items were discussed but are not limited to: Staff files maintenance, medication forms and instructions, attendance documentation, lesson plans, classroom equipment (round table, rocking chair), small playground - cement and rubber unitary surface maintenance.
