Noncompliance Area: Continued Non-Compliance: A renewal inspection was conducted on 10/2/2025. At that time, certification representative observed unlabeled cots in both the Young Toddler Play Space and in the Young Toddler Room (Small). The facility's acceptable plan of correction indicated the correction would be implemented by 10/2/2025. Certification representative conducted an unannounced monitoring on 11/13/2025. At that time, an unlabeled pack-n-play was observed in the Young Toddler Room (Small). Additionally, staff reported that children use two mats in the Young Toddler Play Space which were both unlabeled.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: An unannounced monitoring was conducted on 11/13/2025. At that time, staff person #1 left their group unsupervised in the preschool room while they answered the front door, which is across the facility.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in their assigned group. The staff person shall be physically present with the children in their group on the facility premises and on facility excursions off the facility premises. TIERED LIS 1. Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. The operator shall provide a correction date for when the training will be completed. 3. The legal entity must revise the facility's existing supervision policy to include the definition of supervision found in Chapter 3270.4, procedures for assigning children to a supervision group, procedures for staff transitions, and transition times such as visiting the bathroom. The policy must be revised to include the correct age of a toddler. The policy must be created after staff person #1 completes the Better Kid Care training entitled "Supervision: What's Required". The legal entity must receive approval of the policy from the Regional Office. Once approved by the Regional Office, the legal entity must review policy with all staff and have staff sign off that the understand and will follow the supervision policy. The operator shall provide a correction date for when the policy will be reviewed with all staff.

Noncompliance Area: An unannounced monitoring was conducted on 11/13/2025. At that time, staff person #3 was observed to be caring for children unsupervised. Staff person #3 has not completed the following pre- service training required prior to caring for children unsupervised: pediatric first-aid/CPR.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre- service trainings are completed, staff person #3 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person #3, staff person #3 may not work in a child-care position at the facility.

Noncompliance Area: An unannounced monitoring was conducted on 11/13/2025. At that time, operator reported that staff person #2 is employed at another childcare center. Staff person #2 is at this facility 4-5 hours in the afternoon Monday through Friday, totaling 20-25 hours per week. Operator stated that staff person #2 arrives anytime between 12:30P-1P and leaves around 5:30P. Operator reports that the facility is open on Saturdays, but that staff person #2 does not work on Saturdays

Correction Required: A director shall be employed by a facility and be present at the facility site a minimum of 30 hours per week. TIERED LIS: 1. Shelves were not bolted and were easily moved. Rep observed children pulling on and shaking wooden shelves during inspection.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.
