Inspection Date Reason Description Status
2026-05-26 Unannounced Monitoring 3270.166(4) - Bottles labeled Compliant - Finalized

Regulation: 3270.166(4)

Description: Bottles labeled

Noncompliance Area: Continued Non-Compliance: During the renewal inspection on 5/5/26, there were bottles for multiple children in the refrigerator in the Infant 1 room that were not labeled with the child's name. An acceptable plan of corrections was submitted that stated "A message was sent to families on Wednesday, May 7, 2026, reminding them to label infants' bottles prior to arrival at the center. If bottles arrive unlabeled, teachers have been instructed to ask parents to label the bottles using the supplied Sharpie marker or, if the parent is unable to do so, staff will label the bottles immediately upon receipt using the provided Sharpie to ensure compliance." The correction date was 5/7/26. On 5/26/26, the bottes of child #1, #2, and #3 were not labeled. Going forward, the infant teachers will check all bottles upon arrival each day to ensure they are properly labeled with the infant's name. Additionally, if bottles are not labeled, teachers have been instructed to immediately label the child's name on the bottle using the provided Sharpie marker. Staff will also be reminded of this regulation and procedure during the staff meeting scheduled for May 21, 2026, to ensure ongoing compliance.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Provider Response: (Contact the State Licensing Office for more information.)
The bottles were labeled.
2026-05-05 Renewal 3270.106(a) - Clean, age appropriate Non Compliant - Finalized

Regulation: 3270.106(a)

Description: Clean, age appropriate

Noncompliance Area: During the renewal inspection on 5/5/26, there were sleeping mats being used in the Infant 1 room that were not labeled for use by a specific child. In the Infant 2 room, cots were assigned for use for multiple children on different days.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Provider Response: (Contact the State Licensing Office for more information.)
A labeled grid was created and posted within the classroom; however, the labeling of each child's assigned resting equipment was inaccurate. Although the classroom contained the correct number of mats and cribs based on enrollment, the teachers failed to accurately reflect the appropriate number of beds in relation to the number of children enrolled in the infant classroom on the grid. A conversation was held with the classroom teachers regarding how to properly assign and document cribs and mats for each child enrolled in the classroom.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Provider Response: (Contact the State Licensing Office for more information.)

Teachers were reminded that, in accordance with regulations, there must be at least two feet of space on at least three sides of each mat, and that items cannot be stacked between mats in order to maintain compliance.

Regulation: 3270.124(c)

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Provider Response: (Contact the State Licensing Office for more information.)
All emergency contact information will be maintained and readily available within the classroom where children are receiving care. The child was visiting another classroom at the time of the state inspection, and the teacher inadvertently failed to transfer the emergency contact information to the visiting classroom. As soon as it was identified that child #1's emergency contact information was not present in the classroom, it was immediately retrieved from the Infant #1 classroom and placed in the Infant #2 classroom bag to ensure compliance.

Staff were also reminded that hands must be washed after each diaper change prior to reapplying gloves for the next diaper change. Additional discussions will take place during our upcoming staff meeting, through one-on-one conversations, and during ad-hoc classroom visits to reinforce expectations and ensure ongoing compliance.

Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed.

Provider Response: (Contact the State Licensing Office for more information.)
Although the bins were initially placed in front of the emergency door to prepare the children for self-help skills related to retrieving and storing their nap items before and after rest time, the teachers promptly removed the bins from that area.
