Description: Clean and good repair

Noncompliance Area: The covering on the soft furnishings in the toddler area on the lower floor were split exposing the foam. The paint on the crib in the young toddler room was scraped off exposing the bare wood.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: The health assessment on file for child #2 was dated 8/25/26, which is greater than 60 days of the first day of attendance (see code sheet for start date). An immunization record was submitted within 60 days, but not a health assessment.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Description: Age and Training

Noncompliance Area: Staff #1 did not complete Pediatric 1st Aid/CPR prior to the expiration of the previous training. Staff #1 completed Pediatric 1st Aid/CPR on 2/26/23 with an expiration date of 2/2025. The next Pediatric 1st Aid/CPR was taken 8/30/25.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

Description: Building Surfaces

Noncompliance Area: The fence between the big playground and the small playground was not attached and sitting in the play area. There were exposed screws in the wall above the children's soft sofa in the library area of the second floor classroom.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Description: Supervised at all times /Staff assigned to specific children

Noncompliance Area: On 4/29/26 at approximately 10:00am, Staff #1 arrived to take over the child care duties in the classroom. Shortly after arriving, Staff #1 began lining the children up to take them outside. At that time, it was discovered that child #1, who was now being supervised by staff #1, opened the gate of the second-floor classroom, went down the stairs, and exited the door to the driveway and attached parking lot of the business next door. The child ran onto Penn Ave before a customer at the store next door retrieved him and returned him to the facility.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times and staff shall be physically present with the children in their group on the facility premises and on facility excursions off the facility premises. This portion of the plan must have an immediate correction date. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. The operator shall provide a date for when this training will be completed. 3. The Certification Representative will conduct unannounced monitoring visits over the next three months. During this time period the facility shall have no occurrences of non-compliance relating to supervision. The correction date for this portion of the plan shall be no sooner than 8/7/26.

Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: The facility reports that there are 85 children enrolled with a maximum number of children served at any one time as 76. This requires 2 group supervisors. The facility only has one staff qualified as a group supervisor.

Correction Required: In a facility in which more than 45 children are enrolled, a group supervisor is required for the first group of 45 enrolled children and for each group or partial group of 45 additional enrolled children.
