When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. The language and book centers in space # 7 did not have three related types of activities accessible to the children during today's visit.

Equipment and furnishings were not sturdy, stable and free of hazards. Several mats throughout the center have sections that are beginning to tear or have open cracks exposing foam to the children. The teacher storage cabinet in space # 8 is missing a hinge at the bottom right section which does not allow the door to close properly.

Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The gate has a large opening at the bottom with metal links protruding where children could be injured.

Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. All walls and ceilings including doors and windows were not kept clean and in good repair. Paint is peeling in small areas in two classrooms, the laminate on the cabinets under the sink, has peeled and leaves particle board exposed. There is also a vinyl child sized sofa with multiple tears and vinyl exposed.
