Noncompliance Area: During the renewal inspection on 10/10/24 the file reviewed for child #1 contained a fee agreement that was not reviewed/updated with 6-month time period.

Correction Required: An agreement shall be signed by the operator and the parent. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: During the renewal inspection on 10/10/24 documentation was not provided confirming that annual emergency drill was conducted.

Correction Required: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility.

Regulation: 3290.91(a)

Description: Stairs, exits, etc. unobstructed

Noncompliance Area: The rear exit was obstructed by an activity table.

Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. . Reposition the table or remove the table and ensure that all stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit remain unobstructed.

Noncompliance Area: The rear exit was obstructed by a pile of clothing and other items.

Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed. Remove the items and ensure that all stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit remain unobstructed.

Noncompliance Area: One child's records did not contain a child service report that had been completed within the past 6 months. Complete and submit a current up-to-date child service report for the indicated child. (#2)

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e). Ensure that child service reports are completed for all children as specified in this regulation.

Noncompliance Area: Medication was administered to one child and the medication log incorrectly listed the time of the medication to be administered. It was not consistent with the medication label. (#1)

Correction Required: A medication log shall include the time of administration. Complete and submit a corrected medication log for the indicated child. Revise and submit written policies and procedures regarding the administration of medication to ensure proper administration and the health and safety of the children. The revision must include proper completion of medication logs.

Noncompliance Area: On 1/7/19, at approximately 1pm, staff person #1 was observed caring for seven (7) related and unrelated infants and toddlers exceeding the maximum number of no more than five (5) related and unrelated infants and toddlers at any one time. One infant and 6 toddlers were observed. The ages of the children were verified by staff persons present.

Correction Required: The operator may provide care to no more than five related and unrelated infants and toddlers at any one time. No more than two related and unrelated infants may receive care at any one time. If one infant is in care, four toddlers are permitted. On 1/7/19, at approximately 1pm, staff person #1 was observed caring for seven (7) related and unrelated infants and toddlers exceeding the maximum number of no more than five (5) related and unrelated infants and toddlers at any one time. One infant and 6 toddlers were observed in care. The ages of the children were verified by staff person present. All facility staff person must receive DHS approved training in staff/child ratios for Family Child Care Homes. Submit written verification of the completed training.
