Noncompliance Area: Two staff persons records did not contain verification of completed required CCDBG training (Pa Update) (#1 and #2). Staff person cannot work unsupervised with children until the PA Update has been completed.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Submit written verification of the required CCDBG training (PA Update).

Correction Required: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises. Revise and submit a copy of the facility emergency plan to include a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises.

Noncompliance Area: The facility's emergency plan did not provide for a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children.

Correction Required: The facility shall have an emergency plan that provides for a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children. Revise and submit a copy of the facility emergency plan to include a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children.

Noncompliance Area: One staff person's record did not contain a health assessment completed within the past 24 months. Obtain and submit a current up-to-date health assessment for the identified staff person. (#1)

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. Ensure that health assessments are completed within the required timeframes.

Noncompliance Area: One child's record did not contain a health report completed within the past 6 months as required for a child from infant to young toddler age. (#1)

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler. Obtain and submit a current up-to-date health report for the indicated child. Ensure that all children's records include a current up-to-date health report as specified in this regulation.

Noncompliance Area: One child's record did not contain a current up-to-date health report that had been signed by a physician, physician's assistant or a CRNP. (#2)

Correction Required: A health report must be written and signed by a physician, physician's assistant or a CRNP. The signature must include the individual's professional title. Obtain and submit a current up-to-date health report that contains the signature and professional title of the physician, physician's assistant or a CRNP. Ensure that all children's records include a properly signed health report as specified in this regulation.

Noncompliance Area: One child's record did not contain emergency contact and financial agreement information that had been updated within the past 6-months. (#1)

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.Obtain and submit a written copy of the current up-to date emergency contact and financial agreement information for the identified children. Ensure that this information is updated every 6 months for all enrolled children as specified.

Noncompliance Area: One child's record did not contain the parent's signature indicating written consent for emergency medical care and for administration of minor first-aid procedures by facility staff. (#2)

Correction Required: A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission. Noncompliance Area: Children's records are missing release person's name.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Complete emergency contact information does not accompany children on excursions.

Correction Required: When children leave the facility on walking and riding excursions, emergency contact information specific to each child on the excursion shall accompany a staff person on the excursion.

Noncompliance Area: Staff person #1 does not have two references.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person

Noncompliance Area: Group wants to operate on the first floor of a row house. The floor plan is straight through and open from living room to kitchen. There is not a wall between the kitchen and child care space that covers more than 50% and separates the kitchen as a separate room. The kitchen contains a stove and oven, a heat source. Capacity space cannot be measured since it includes a kitchen in child care space.

Correction Required: Measured indoor space excludes space occupied by halls, bathrooms, offices, kitchens and locker rooms.

Noncompliance Area: During renewal inspection on 5/9/17 certification representative observed the first aid kit stored in a plastic bin on the floor which is within access of the the children.

Correction Required: A first-aid kit shall be inaccessible to children.
