Initial Comments: The following deficiencies were observed at the time of the Initial Monitoring Inspection conducted on 3/27/2026 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The following was discussed but not limited to: 1). Ensuring that toilet fixtures are maintained in a clean condition. 2). Ensuring that personal items are labeled with the child's first and last name.

Initial Comments: The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 12/10/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The fingerprint clearance cards for 24 of 24 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1) Ensuring that outside water fountains are kept in a clean condition. 2). Ensuring that crack sink caulking is repaired. 3). Ensuring that the resilient surfacing wood chips are the required 6 inches. 4). Ensuring that the field trip form has the date and the hourly check-ins completed. 6). Ensuring that parents/guardians sign the children out with a time and signature. 7). Ensuring the Emergency, Information, and Immunization cards are fully completed. 8). Ensuring that parents/guardians are detailed and specific when completing the medication authorization form. 9). Ensuring that when people from other businesses are smoking outside, enrolled children are not in the vicinity.
