Initial Comments: The following deficiencies were observed at the time of the initial monitoring inspection conducted on 01-08-2026, and are subject to change pending programmatic review. A full inspection was not conducted at this time. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please email the Department a picture of the license plate attached to the vehicle that was missing one. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The following was discussed, but not limited to: 1. New rules discussion (effective: 08-03-2025) 2. Children's emergency record documentation. 3. Staff fingerprint card verification via DPS. 4. Staff file dates of hire. 5. Maintenance of licensed vehicles.

Initial Comments: The following deficiencies were observed at the Initial Inspection conducted on 10/8/2025, and are subject to changes pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 16 of 16 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following was discussed, but not limited to: 1) Ensuring Emergency cards in the Transportation binder are updated when the main Emergency cards are updated, 2) Tummy time rules, 3) Infant feeding instructions updated when changes occur, 4) The following staff file information is required to be updated effective the date of change of ownership (date of employment, TB attestation, DCS clearance, DPS fingerprint clearance card verification, Criminal history affidavit, and orientation training, and 5) Submit outdoor plans showing the separation of the square footage of 2 outdoor activity areas.
