The file lacked documentation verifying that the licensee had contacted the Department of Public Safety within seven working days to verify that the fingerprint clearance card is valid.

Findings: The Surveyor determined the file for staff #2 lacked documentation of the licensee's submission of the staff's Central Registry Background Clearance check. NOTE: Staff #2's file was also cited for this deficiency at the time of the previous compliance inspection that was conducted in 8/2017.

Findings: 1) The file for staff #3 (date of employment- 5/20/2016) lacked documentation of current training and it could not be verified if the staff had completed any of the required 18 hours of annual training for the period of 5/20/2016 through 5/20/2017; and 2)The file for staff #4 (date of employment-11/17/2014) contained documentation of training and indicated that the staff had completed only 6 of the required 18 hours of annual training for the period of 11/17/2016 through 11/17/2017.

Findings: During an inspection of the toddler room's bathroom, the Surveyor observed that the toilet tissue was not mounted and the roll was placed on top of the dispenser.

Findings: Finding: The Surveyor observed that the Accident and Emergency plan in the toddler room and room B were dated 7/2016 and 8/2016, respectively, and it could not be verified that the plan had been updated within the last 12 months.

Findings: During an inspection of the toddler room's bathroom, the Surveyor observed a second soiled diapering counter, in addition to the one that was stored in the room's diapering area. The staff acknowledged that soiled pullups are changed in the bathroom for the children who are being toilet trained. During a discussion with the Director regarding this deficiency, the Director stated that at the beginning of the school year, staff had been changing 2 children's pullups in the bathroom for preschool age children, since the children were not toilet trained at that time.

Findings: The Surveyor observed the following hazardous in the toddler's outdoor activity area: 1) Five 2 X 4 pieces of wood stored in the corner. Three of the pieces of this wood had rough, splintering pieces of wood; and 2) Both entrance gates to the wrought iron fenced enclosed toddler playground area had been secured shut with bungee cord clips, while the children were inside this fenced area. The toddler staff told the Surveyor it was to prevent the children from getting out of this area. Later during a discussion with the Director, the Director stated that it was required to be done, per the licensee's policy (the "Pre-opening checklist"). The Surveyor explained that the children could not be secured into an area, without the children being able to get out of the area in case of emergency.
