Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 8/15/2025 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Empower Self-Survey was emailed to the director following the inspection. Please complete it within 10 days of receipt. The DES Group-Size contract form was completed at the time of the inspection. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: *Ensure immunization exemptions are completed. *Ensure cleaning tools (brooms) are inaccessible.

Initial Comments: The following deficiencies were observed during the Compliance inspection conducted on 8/20/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. 4 of 4 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. The cracked rubber surfacing on the playground. 2. The divot in the astro turf on the playground. 3. Keeping drinking cups accessible to enrolled children. 4. Storage of blankets on mats. 5. A heat sealed mat is required for special needs diaper changing in the toilet room. 6. Storage in toilet rooms. 7. Children’s Record Cards must contain the required information. 8. Exposed foam on the brown vinyl chair. 9. The person responsible to administer medications. 10. Child Care Careers staff files must be complete. (Criminal History Affidavit and verification of fingerprint cards.)
