Findings: Per allegation and based on the Surveyors' observations, upon the Surveyors arrival to the facility, a Staff member did not supervise each enrolled children at all times. Staff #1 was in the doorway of the classroom with 14 children in the Four's classroom and 1 child was across the hall in the restroom.

Findings: Based on facility documentation, staff interview and the Surveyors' observations, the facility lacked diaper changing logs for the first week of November- 2021. Staff stated that the facility switched from bright wheel back to individual daily logs and were not keeping diaper logs.

Staff stated they were not yet potty trained and they changed the children in the restroom. The Three's classroom lacked an approved diaper changing area.
