Findings: Based on Surveyor's observation during the inspection of the diaper changing areas, the following was noted: *1 year old room-The soiled diaper container was not equipped with a liner and the staff stated the lid was missing for the container; and *2-3 year old room- The soiled clothing container was not equipped with a liner. The lid to the soiled diaper container was laying behind the container and there was soiled diapers in it.

Findings: Based on the Surveyors' observations, it was determined that the diaper changing surface was not kept clear of unrelated items as follows: *The Infants' classroom diaper changing surface had a radio, a tablet, a lanyard with keys, a pen, and a mounted paper towel dispenser on the surface; and *The 2's classroom diaper changing sink had blue and pink speckles of paint along with other brown debris throughout the sink and in the drain.
