Child-Parent Centers was founded in 1967 in Tucson Arizona. We provide Head Start services to children and families in Pima, Cochise, Graham, Greenlee, and Santa Cruz counties. Head start is a comprehensive federal program administered by the Office of Head Start, within the Administration of Children and Families of the Department of Health and Human Services. Findings: During the review of enrolled children's sign in/out records, Surveyor 1 noted that Child 1 was signed in with a first name and last initial on 1/8/2019.

Findings: Upon review of the children's emergency cards, the Surveyor observed that Child #1 was missing the name of the individual to be contacted in case of injury or sudden illness of the child.

Findings: Upon review of the children's attendance records, the Surveyor observed that Child #2 was being signed in and out with initials only in February 2018.

Findings: The Surveyor observed that although the medication belonging to Child #4 was prescribed as a two- pack, there was only one in the box.

Findings: Upon review of the children's medication logs, the Surveyor observed that Child #3 was administered medication on 12/6/17 and 12/12/17 without the signature of the staff member who administered the medication.

my son attended Sunnyside head start and we loved it then he is now going to be 11 and my daughter just turned two and I plan on her attending Sunnyside head start as well.
