Findings: The Surveyor observed that the fire extinguisher in the 1 year old room had a tag that was dated 2017. All extinguishers must be serviced and tagged at least once every 12 months.

Findings: The Surveyor reviewed the attendance rosters and found the 1 year old room had 5 children in the room, and only 2 listed on the attendance roster. The preschool room had 10 children in the room, and only 9 on the roster.

Findings: The Surveyor observed 19 children in the facility with the youngest child being 1 years old. Three staff were observed in 3 different rooms. The facility did not have sufficient staff to perform all duties. When questioned how staff meet ratio when needing to use the restroom, Staff 1 stated that we can go in here (pointing to the bathroom in the classroom). The Surveyor informed Staff 1 that staff may not leave a room out of ratio and supervision, even when the bathroom is in the classroom.

Findings: The Surveyor observed that the diapering area in the 1 year old room did not contain a soiled clothes container, or a soiled diaper container. The 2 year old room did not have a soiled clothes container.
