Findings: * Surveyor reviewed ten children's emergency records. The following information was missing: * Child 1-3 - Date of enrollment. * Child 4 and 5 - The name and contact telephone number of one additional individual authorized by the child's parent to collect the child from the facility in case of emergency, or if the child's parent cannot be contacted.

Findings: a) Upon arrival to the facility, Surveyor pressed the buzzer twice, knocked on the door twice, and finally pressed the buzzer for several seconds before an individual came to the door. The individual stated that the buzzer is connected to the Director ' s office and the door was closed which prevented them from hearing the buzzer. The Director was not present at the facility at beginning of the inspection.

Findings: a) The Director was not present at the beginning of the inspection. Surveyor reviewed the chain of command and observed Staff 1 listed as the director designee; whom according to the staff has not been employed at the facility for over a year. b) May also be cited under R9-5-301.B.1.

Findings: a) Surveyor observed a medication for Child 1 that did not have written parental permission to administer the medication. b) The medication for Child 2 expires in October 2019, the documented ending date of the dosage period was May 2020. c) The medication for Child 3 expires in May 2020, the documented ending date of the dosage period was June 11, 2020. d) The medication for Child 4 expires in March 2020, the documented ending date of the dosage period was May 12, 2020. e) The medication for Child 5 expires in February 2020, the documented ending date of the dosage period was May 12, 2020.

Findings: a) Surveyor observed a child climb onto the white mission style structure on the studio playground. The child climbed to the top the structure which is the same height of the exterior fence of the playground. According to staff, the children are always using the structure as a climbing structure, which is not its intent. (a picture of the structure was taken)

Findings: a) Surveyor observed bottles of sanitizer and disinfectant spray underneath the sink in classroom D. Both bottles had a child warning label, and both were accessible to the enrolled children.

Findings: a) Surveyor observed that the lesson plan in classroom D was dated 10/8/18.

Findings: a) Surveyor observed that the lead teacher in each classroom administers medications to children assigned to their specific classrooms. If the lead teachers are absent, the co-teachers are responsible for administering the medications. If both teachers are absent, the Site Supervisor administers the medication.

Findings: a) Surveyor observed medication being stored in a cabinet underneath the sink in classroom D. The medication was accessible to the enrolled children.
