Initial Comments: The following deficiencies were found at the time of the Compliance inspection conducted on May 11, 2026, and are subject to changes pending programmatic review. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. Two of two fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please complete the Plan of Corrections via the online Portal within 10 days of receipt of this Statement of Deficiencies. The DES group size was observed to be compliant at the time of the inspection. The Emergency Disaster Contact form was completed following the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. Items discussed, but not limited to, were: -Storing items in the bathroom, -Renewing license via the portal - Anniversary Application in "Applications" or "Application History", -Renew license before 6/27/2026, -Review all staff files for expiring fingerprint cards.

Initial Comments: Unable to complete the Initial Monitor inspection on August 21, 2024. Compliance Officer 1: Laurie Mckenna Compliance Officer 2: Katie Corrow Upon arrival to the facility, it was observed by both Compliance Officers that the front windows and entrance to the facility were covered with metal barriers. Compliance Officer 1 contacted the facility's Designated Person and Owner by phone from the facility parking lot. The Owner reported that they had not yet been able to open the facility due to a delay of the approval of a DES contract. An email notifying Compliance Officer 1 that the facility would be temporarily closed until further notice, was sent on August 22, 2024.

Certificate of Liability Insurance: expires on 10/24/2024 Fire Inspection: 6/18/2024 Dept of Health: Kitchen, expires on 12/31/2024 Items discussed but not limited to: -No storage of non related items in bathrooms, -Renewing license via the portal - Anniversary Application (in "Applications" or "Application History"), -Review all staff files for expiring fingerprint cards, -Mats, sheets and blankets, -Exits unobstructed, -Shelving, -Choking hazards, -No walkers in infant rooms, -Diaper surfaces, -Handwashing, -Water temperature at diaper changing sinks, -Exit maps, -Labeling personal products, -Activity schedules, -Modified diets. The Facility was not licensed at the time of inspection.
