Determined During Course of Investigation Complaints
Plan: 2.) Personnel use physical actions toward a child: In the staff meeting, I emphasized to the teachers that physical punishment is not allowed. Should any staff member be found to have harmed a child, termination will be immediate. We will conduct monthly training sessions covering policy and discipline, as well as stress management and related topics.

Plan: 3.) Personnel restrained children in high chairs for a long period of time: High chairs have been removed from the classrooms for two-year-olds and older. They are not to be used as restraints for children and will only be allowed in the infants' and toddlers' areas during meals and manipulative activities.

Determined During Course of Investigation Complaints
Plan: 4.) Personnel did not maintain supervision at all times: The classroom for two-year-olds was promptly moved to a space that included a sink within the room rather than one on the side. This ensured that the staff could always keep an eye on the children while washing their hands and maintaining supervision at all times.
Description: Additional Non-Compliance Found During Investigation: 4.) *SERIOUS* Supervision: Personnel did not maintain supervision at all times.
2025-02-26 340:110-3-288(c)(2)(C)
Substantiated Complaints
Plan: 1.) Personnel shouting in a child's face: The teacher was taken out of the classroom and suspended for two days. The director and the assistant director talked with her about the reasons for the suspension, and she was told that she would not be permitted to be alone in the classroom until approved by licensing. A staff meeting was held immediately, during which everyone was updated on the situation. I am increasing my classroom visits, reminding the staff about inappropriate disciplinary actions, such as shouting at a child and using harsh language. To avoid similar incidents in the future, teachers are encouraged to call the front office to step out of the classroom for a breather, and longer breaks have been made available.
Description: 1.) *SERIOUS*Discipline: Psychological and emotional - Personnel yelling in a child's face.
