Findings: SG completed the basic health and safety training March 2022, but it was due November 2021. Director reports asking the staff to complete the training and giving the staff written notices to complete the training. Lic. Spec. observed the written notices given to new staff. The non-compliance was corrected when SG completed the basic health and safety training. Please continue to encourage and assist staff to complete the required trainings timely.

Findings: RY needed to complete 6 clock hours of training for her September 2019-September 2020 training year and only completed 4.5 clock hours. RY was late completing training for her 9/2019-9/2020 year as well as the 9/2020-9/2021 training year. RY completed 3 clock hours during her 2020-2021 training year which was counted towards her 2019-2020 training year. This left her with only 1.5 clock hours for her 20-21 year. RY completed 12 clock hours of training on 3/23/2022. RY is now up to date with her required trainings for her 19-20 year and her 20-21 year and this current year September 2021-September 2022! Please continue to remind staff to stay on track with their trainings so the trainings are completed in a timely manner. This non-compliance was corrected on 3/23/22 when RY completed 12 hours of training.

Director notifies and reminds the staff via written notices to keep up with their training requirements in a timely manner. The non-compliance was corrected when HT completed the training. Please continue to encourage and assist staff to complete the required trainings so that the trainings are completed on time.

Findings: Upon arrival Lic. Spec. observed a group of 13, four and five year olds with1 staff person. This size group and age group requires 2 staff present. The qualified staff could have requested an additional staff before the 11th child arrived. The director typically is in this classroom at this time, but the director had a personal emergency which delayed her arrival. There were actually extra staff available in the building. An aide, EM did come into this classroom by 8:17am thereby correcting the non-compliance. It is unclear how long the staff child ratio was out of compliance- it was likely no more than 30 minutes. Please be sure that staff child ratios are correct throughout the day.

Findings: There is one aide, RY who didn't complete the 6 hours of training for her training year of September 2019-September 2020 timely. She has evidence on site of 4.5 hours of training which was completed between November 2020 and now. This training was late and will be used to meet her 2019-2020 year of training requirement. The director states that RY did complete the aide orientation training @ care courses but that certificate is not available at this time. When that 3 hour training is verified then that will satisfy her training requirement for the 2019-20 year. This aide will need to complete additional training by September 2021 to equal 6 hours of training. It was also noted that the Aide orientation was to be completed by March 2020 by RY as that was 6 months after her hire date. Please forward written corrective action within 30 days.

Findings: Child fell and scraped forehead on wooden fence. Child was taken to the hospital the following day. Child had abrasion on forehead and 2 scratches. (pictures observed) This occurred on 9/11/17. The Region 10 OOC Office was not notified of the injury w/in 24 hours.

Findings: Not all new hire staff were reported to Office w/in the required time frame. Director stated she was told not to fax this type of information. Advised on 5-2-18 that it would meet the required timelines if information was faxed to Office or may scan information to Licensing Specialist's email. Director has been forwarding original releases to Office after faxes. The Office does not require original Personnel change form, the fax copy is acceptable.
