| Findings: TM, who meets the birthdate requirement for lead blood testing and is under 6 years of age, did not have blood test results on file during inspection today. The Director was able to locate the child's Part 2 health inventory form in the nurse's office and added a copy to child's child care file, but the form only indicated that the lead screening had been done. In accordance with COMAR 10.11.04, the child should have had the lead blood test results submitted to the program-school within 20 days of the first day of enrollment. Please obtain lead test results or documentation from the child's health care provider that testing is not needed and add to child's file. Please send copy to Licensing Specialist. | |||

| Findings: Director stated she keeps a personal record of when she is not present in the center and that a record of which staff members are working at which site is kept by the child care coordinator. There is no written record present onsite today showing days on which a substitute provided care and for which staff member the substitute worked (Director was out yesterday). Licensing Specialist will send Director a blank substitute record template to keep at the center. Director should advise any substitute to use this substitute record when onsite and substituting for another staff member who is out. | |||

| Findings: Licensing Specialist did not observe orientation forms for 1 substitute who has worked at this site. When a substitute is onsite, Director should review orientation process and have each substitute sign an orientation form and add to their file. When program uses staff from other sites as substitutes, Director should insure those staff have been oriented to this site and have a signed orientation form in their file. Send written plan of correction to Licensing Specialist. | |||
