I will reach out this child's guarding for their immunization records or a letter of exemption. Please note that I have been unable to get the shot records for the two Doumbouya children after speaking with mom several times and calling the doctor's office and they still haven't provided it. I was told to put in the request via fax after calling them. Which I did. But they still haven't sent it. I informed the parents that the children can not continue care until they have provided the shot records. Please see the request for the shot records to the doctor's office attached.
