All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A key to a locked cabinet, with disinfectant wipes that were stored below five feet, was stored less than five feet high from the floor.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. The Phytoplex Anti-fungal cream was stored in an unlocked cabinet above five feet with diaper creams in space #1, the classroom with infants.

Questionable medication instructions from parents were followed without signed written dosage instructions received from physician or authorized health professional. The Phyytoplex Anti-fungal cream has been used on a child under the age of 12 months and the medication stated a warning to not use on children under two years of age unless directed by a doctor. A doctor’s note was not on file for a child under the age of 12 months, only the medication authorization form signed by the parent.

Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. The staff medical report for PH had a questionable answer on the medical report that needs to be clarified. The question for is the individual physically and emotionally fit to care for children was answered “no.”
