Children were observed playing throughout the playground area and areas were staff members were not positioned.

Staff reported two (2) children were climbing on the easel on 8/20/24 at approximately 2:10pm when one (1) child got their foot caught at the top of the approximately four (4) foot easel and fell to the ground, causing the child to break their arm. During the visit today, a child was observed climbing on the easel and jumping off the easel ledge, where art supplies would be stored, and to the ground. The same child was observed climbing onto the window ledge of a playhouse and jumped to the ground. The incidents that occurred created an unsafe outdoor environment.
