Noncompliance Area: The file of child #1 lacked an updated health assessment. The only health assessment on file was dated 1-25-19 and an enrollment date of 1-28-19.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: The file of staff #1 with a start date of 3-5-04 had a National Sex Offender Registry document dated 10-27-20. The file of staff #2 had a Child Abuse Clearance dated 10-9-15. At the time of reviewing the files with the provider it was determined there was not an updated Child Abuse Clearance on file for staff #2.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: The facility lacked a posted written plan of daily activities.

Correction Required: A written plan of daily activities and routines, including a time for free play shall be established for each group. The plan shall be flexible to accommodate the needs of individual children and the dynamics of the group.

Noncompliance Area: The file of child #2 lacked an emergency contact form with the full address of the child's physician or source of medical care.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Noncompliance Area: The file of child #2 lacked an emergency contact form with the policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: The files of children #1 and #2 lacked emergency contact forms with the full addresses of individuals designated by the parent to whom the child may be released.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: The facility lacked a posted written notification of safe routes.

Correction Required: Written notification of safe routes shall be posted by the operator at a conspicuous location in the child care facility.

Noncompliance Area: The file of child #1 lacked documentation of signed parental consent for administration of minor first-aid procedures by facility staff.

Correction Required: A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: Observed a Bumbo brand seat that was recalled due to the fact that it lacked the secure straps.

Correction Required: Children's toys and equipment, including furniture and rest equipment, described as hazardous by the United States Consumer Product Safety Commission may not be used by children at the facility and may not be on the premises at the facility. At the time of inspection, the operator shall submit to the Department written affirmation on a form provided by the Department stating that the facility is in compliance with this requirement.

Noncompliance Area: Observed Zinc Oxide diaper rash with an expiration date of 1/2018.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: There was no written parental consent for administration of Zinc Oxide diaper rash for child #1.

Correction Required: A parent shall provide written consent for administration of medication or a special diet.

Noncompliance Area: Observed gauze in the first-aid kit that was not sterile due to the fact that it was not in its original packaging. The gauze was loose stored in the first-aid container.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: The file of child #1 lacked an agreement with the child's arrival and departure times.

Correction Required: An agreement shall specify the child's arrival and departure times.

Noncompliance Area: The files of children #1 and #4 lacked an emergency contact form with the full physical address of the child's physician or source of medical care. The file of child #2 lacked an emergency contact form with the physical address of the child's physician or source of medical care.

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Noncompliance Area: The file of child #1 lacked an emergency contact form with the work address and telephone number of the enrolling parent. The file of child #2 lacked an emergency contact form with the work telephone number of the enrolling parent. The file of child #4 lacked an emergency contact form with the work address of the enrolling parent.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.
