Spoke with child #1 parent and explained that the form we had did not specify about the parents strong belief as to not vaccinate for the influenza . We would need a letter from the parent specifying there strong personal objections. On 11/27/2019 facility received a letter from the parent explaining there strong personal objection . Letter is currently in the child's file. facility will be sure any parent refusing to vaccinate for the influenza vaccination will have a written letter explaining religious belief or personal objections and be kept in the child's file.

On 11/27/2019 got original health assessment form and also a letter from the physician stating physical and tb testing was done both are currently in staff folder. immediate action was taken to contact the first aid trainer. explained I needed a letter of verification from her that she trained all the staff at the facility and that it was written and signed by her. Also explained I needed a copy of her nursing certificate to keep on file. on December 5,2019 Steffanie Rhoades did submit the facility the proper letter from her and verified the training and gave the facility a copy of her nursing license. The facility will be sure that all documents from qualified medical personal are on file and all future documents be written by the instructor signed and dated properly.
