Noncompliance Area: On the date of inspection, it was observed that there was not an operable smoke detector on the upstairs floor because it was missing. Also, it was observed that two of the three fire extinguishers present, none of which were located in the cooking area, were tagged to be beyond their inspection dates and a portable fire extinguisher retrieved from the provider's basement was dated 1984 and had the tape removed from the nozzle, which the instructions on the device indicated was a sign that a new one should be purchased. These conditions at the facility pose a threat to the health or safety of children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. After installing an operable smoke detector on the second floor and obtaining an acceptable portable fire extinguisher, the operator will verify correction to an agency of with an agent of the department.

Noncompliance Area: The operator's file lacks a copy of the previous health assessment.

Correction Required: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: The operator lacks five hours of annual training for the anniversary year 5/12/18- 5/12/19, five training hours attained during this time were obtained to a correct a violation the previous inspection year.

Correction Required: A staff person shall obtain an biennial minimum of 12 clock hours of child care training.

Noncompliance Area: In an unlocked undersink cabinet, grooming products and other items labeled "Keep Out of Reach of Children" were observed. It was disclosed that normally the cabinet is locked when children are in care, but the lock has since broke.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: In the dining room, an area of peeling pain was observed on the wall below the window near an outlet. On the front porch, a large concrete surface was observed to have many areas of peeling paint.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: The most current health assessment in the file for Staff #1 is dated almost 3 years ago.

Correction Required: The facility's current certificate of registration and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same locationThe operator shall post the emergency plan in the facility at a conspicuous location.

Noncompliance Area: The last documented review of the facility Emergency Plan was dated more than one year ago.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: The last documented training that Staff #1 completed is dated approximately a year and a half ago.

Noncompliance Area: A bottle of Windex labeled 'Keep Out of Reach of Children' and a box of Prep-H medication intended for staff was observed in an unlocked, under-sink cabinet in the downstairs restroom.

Correction Required: Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.

Noncompliance Area: The first-aid kit lacked gauze.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: The file for facility person #1, whom is the only person providing care in the family child care, lacked current first-aid training. The first-aid training expired on 2-7-17.

Correction Required: Competence in first-aid is demonstrated by the completion of training by a professional in the field of first-aid. First-aid training shall be renewed on or before expiration of certification or every 3 years, as applicable. One or more persons competent in first-aid techniques shall be at the facility when day care children are in care.
