I have no control over what a doctor fills out or what the parent obtains when getting a child's physical. My solution to this is to expel the child from day care until they take one of the state-mandated physical papers and have it totally completed. Both children have been expelled from the day care until proper state-approved physicals are obtained and delivered to the day care.

This was obtained by the parent on 3/4/20 and I stapled it to his physical. I neglected to send you a copy. He will not return until I can check the records, take pictures and submit them to you. However the flu shot is missing or may now be there. Both children will be expelled from this day care until proper, state-accepted health-approved physicals are obtained and delivered to the day care.
