Noncompliance Area: During renewal inspection on Friday, 02/07/2020, cert rep observed staff not properly supervising children during nap time. Cert rep observed a staff on a cellphone for the duration of nap time.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in 3290.51-3290.52 (relating to maximum number of children; and staff:child ratio).

Noncompliance Area: During renewal inspection on Friday, 02/07/2020, cert rep observed facility failed to have verification of current rabies immunization for pet at facility. Rabies immunization on file expired on 05/19/19.

Correction Required: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.

Noncompliance Area: In review of children's files, child 1 and 2, both enrolled 6 months or more, failed to have a child service report on file at the facility.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: In review of children's files, child 3 file failed to include a fee/service agreement.

Correction Required: A parent shall receive the original agreement. The facility shall retain a copy of the agreement. A child's record must contain a copy of the initial agreement and subsequent written agreements between the parent and the operator. The parent receives the original agreement.

Noncompliance Area: In review of children's file, child 2 emergency contact and agreement failed to be updated within six months.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: In review of children's files, child 1, enrolled date 01/22/19, file failed to include a health report. File only included immunization record.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: During renewal inspection on Friday, 02/07/2020, cert rep was informed that an enrolled infant failed to have a written feeding schedule.

Correction Required: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent.

Noncompliance Area: In review of children's files, child 4 failed to have a record on file at the facility.

Correction Required: An operator shall establish and maintain an individual record for each child enrolled in the facility.

Noncompliance Area: During renewal inspection on Friday, 02/07/2020, cert rep observed facility's emergency plan failed to include evacuation information including a shelter away location.

Correction Required: The facility shall have an emergency plan that provides for shelter of children during an emergency including shelter in place at the facility and shelter at locations away from the facility premises. The facility shall have an emergency plan that provides for evacuation of children from the facility building and evacuation of children to a location away from the facility premises. The evacuation routes and evacuation plans to exit the building may be the same as those required by 3290.94 (f) and (g) (relating to fire drills).

Noncompliance Area: During renewal inspection on Friday, 02/07/2020, cert rep observed facility's emergency plan annual review failed to be documented in writing by operator.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: Staff persons had not been trained on the facility's emergency plan at the time of the renewal inspection on Friday, 02/07/2020

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training. A facility person's record shall include records of training required by the Department.

Noncompliance Area: During renewal inspection on Friday, 02/07/2020, cert rep observed cleaning material and acholic beverages located under sink and lower cabinet in kitchen which were accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: During renewal inspection on Friday, 02/07/2020, cert rep observed several areas throughout the childcare space which contain stacks of boxes, papers and other clutter, causing a safety hazard to children in care.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: During renewal inspection on Friday, 02/07/2020, cert rep observed ceiling in back kitchen area had damaged plaster.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: During renewal inspection on Friday, 02/07/2020, cert rep observed basement exit door, which is used as a means of egress, was obstructed.

Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed.

Noncompliance Area: During renewal inspection on Friday, 02/07/2020, operator was unable to verify to cert rep if fire evacuation drills were held at least four times a year.

Correction Required: Fire evacuation drills shall be conducted at least four times a year.

Noncompliance Area: During unannounced inspection on Tuesday, 12/31/19, cert rep observed facility failed to have emergency contact information for a child on a riding excursion.

Correction Required: When children leave the facility on walking and riding excursions, emergency contact information specific to each child on the excursion shall accompany a staff person on the excursion.

Noncompliance Area: During unannounced inspection on Tuesday, 12/31/19, cert rep observed fire extinguisher pressure gauge indication read empty. This condition at the facility pose a threat to the health or safety of children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Demonstration of Compliance with 34 Pa. Code 403.23 is required. The operator will demonstrate an acceptable portable fire extinguisher to an agent of the department.

Noncompliance Area: During unannounced inspection on Tuesday, 12/31/19, cert rep observed facility failed to have a first-aid kit while on a riding excursion.

Correction Required: One first-aid kit per child care group must accompany children and facility persons on excursions from the facility. Each first aid kit taken on an excursion must contain a bottle of water in addition to the items specified at 3290.73(c). A first-aid kit, including the contents as specified in 3290.73 (relating to first-aid kit) shall be in the vehicle when children are being transported.

Noncompliance Area: During renewal inspection on Monday, 03/25/19, cert rep observed facility failed to have a written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency.

Correction Required: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children.

Noncompliance Area: During renewal inspection on Monday, 03/25/19, cert rep observed facility's emergency plan failed to provide a method of contacting parents at the beginning and end of emergency.

Correction Required: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises. The facility shall have an emergency plan that provides for a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children.

Noncompliance Area: During renewal inspection on Monday, 03/25/19, cert rep observed facility failed to have a fire evacuation plan.

Correction Required: Fire evacuation plans shall be developed and posted. Evacuation plans shall provide for removal of all persons from the facility in a single trip.

Noncompliance Area: During unannounced inspection on Friday, 02/01/19, cert rep observed an infant child sleeping on the floor and cots that were in use by children were not labeled.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: During unannounced inspection on Friday, 02/01/19, cert rep observed cots failed to have the required 2 feet of space on three sides while in use.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: During unannounced inspection on Friday, 02/01/19, cert rep observed child answering door upon arrival to facility, infant child sleeping alone unsupervised on the second level of the home and children being left alone downstairs on the 1st level of the home while operator went upstairs.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: In review of staff files, staff person 1 failed to have results of TB screening contained in file.

Correction Required: An adult health assessment shall include Tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, CRNP, the Department of Health or a local health department. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: In review of staff files, staff persons 1 and 2 failed to have disclosure statement contained in file. Staff person 1, whom was hired on 10/29/18, failed to have PA state clearance and child abuse clearance contained in file.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: During unannounced inspection on Friday, 02/01/19, cert rep observed facility over capacity. Operator had a total of 7 unrelated children (2 infants, 2 preschoolers and 3 young school age children) receiving care at one time.

Correction Required: The number of children in care may not exceed six children at any one time who are unrelated to the operator.

Noncompliance Area: During unannounced inspection on Friday, 02/01/19, cert rep observed surge protector located in childcare space failed to have outlet covers. This facility serves children that are age five and younger.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: During unannounced inspection on Friday, 02/01/19, cert rep observed cleaning materials in bathroom accessible to children and hand sanitizer located in childcare space accessible to children.

Correction Required: Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
