Noncompliance Area: During time of inspection, it was observed that child # 3, who has been attending facility over 6 months did not have a child service report completed and documented in their record.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3280.124(f).

Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: During inspection it was observed that bottles being used for the children were not labeled with children's names. In addition, a child identified as being 3 months old was observed sitting in a baby lounger chair being feed with a bottle propped under a small blanket.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name. An infant 6 months of age or younger shall be held while being bottle fed.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1- may not work in a childcare position at the facility. . A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1-- may not work in a childcare position at the facility.

Noncompliance Area: During renewal inspection on Wednesday, 10/19/22, cert rep observed rest equipment failed to be labeled with each child's name.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

A child's record must contain initial and subsequent health reports.
