A complete inspection of the facility was not conducted. The Plan of Corrections will not be accepted at this time. The following staff-to-children ratios were observed for school-age children: Art room: 1:19 Game room: 1:13 The Department contacted the complainant on 03/19/2025. Interviewed: 3 children A city maintenance employee with the Parks Department. Documents Reviewed: 4 staff files Field Trip Permission Forms for 03/17/2025-03/19/2025 Incident Reports dated 03/18/2025 Countryside Recreation Center Hourly Reporting documents for 03/17/2025-03/19/2025 Daily schedule for 03/18/2025 Proper Use playground equipment warning tag The fingerprint clearance cards for 3 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. Upon completion of the Complaint Investigation (Case 00122997), it was determined from facility documentation, the Proper Use playground equipment warning tag, interviews with the children/city employee, and the Compliance Officers' observations that 3 out of 3 allegations were substantiated.
