A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. 842 in space #5, In space #5, a permission to administer Aquaphor diaper cream form expired on 7/1/23 and in space #10 a permission to administer “thinkbaby” sunscreen expired on 3/8/23. Teachers should check Permission to Administer Medication Forms when brought into the facility and regularly thereafter to ensure they are filled out completely and properly and to ensure permission has not expired. A parent may give standing authorization for up to 12 months to apply over-the-counter, topical ointments, topical teething ointment or gel, insect repellents, lotions, creams, fluoridated toothpaste, and powders, such as sunscreen, diapering creams, baby lotion, and baby powder, to a child, when needed. I suggested that Permission to Administer Medication Forms be kept with the mediation and that staff continue to check all medications and Permission to Administer Medication Forms at least once a month to prevent further non-compliance regarding medication.

One employee, hired 8/16/21, did not have a signed medical on file until 9/13/22.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. An aerosol can of Coppertone sunscreen was in unlocked storage in space # 9.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space #1, emergency medications were stored on a shelf less than five feet from the floor.

A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space#1 permission to administer an inhaler expired on 4/5/23 however, the medication continued to be administered through 5/25/23.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space #1 an Epi-pen expired on 3/2023 and was not returned to the parent with 72 hours. This medication was still at the facility.
