Findings: Based on Surveyor's observation, review of facility documentation and information provided by the staff at the time of the inspection, it was determined that the following staff files lacked the required information: Staff #1 (Start date: 7/16/2012): Proof of a negative Mantoux TB test. Staff #2 (Start date: 4/4/2017): The staff had a TB test that was read on 3/20/2017 but the result did not read negative. (Reference R9-5-301.F)
