Initial Comments: There were no deficiencies observed at the time of the Initial Monitoring Inspection conducted on 3/23/2026 and is subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was given to the facility at the time of the inspection. A full inspection was not conducted. The DES group size was observed in compliance at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Children one year old and walking will not be mixed with enrolled infants when more than six children are present in the facility *Supervision *Sanitation Permit to Operate *Toilet seats will remain secured and toilet rooms free of hazards *Resilient surface manufacturer recommendation for the fall zone of the airplane teeter totter

The fingerprint clearance cards for 9 of 9 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following information was discussed but not limited to: *Staff file document updates *Safety covers on round door knobs can be used on doors that are designated as an exit *Staff qualification (training hours) will be available *Staff education documents will be available *A copy of the back of the fingerprint clearance card will be available *Designated exits will be unobstructed
