Initial Comments: There were no deficiencies observed at the time of the Annual Compliance Inspection conducted on 10/1/2025. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: *Discussed the new rule set and the location of the new forms. *Discussed the requirements for a foreign transcript or diploma and what a staff member needs to be alone with enrolled students. *Reminder that the caulking in Stall #1 and Stall #3 of the boys' toilet room needs to be redone.

Initial Comments: There were no deficiencies observed at the time of the Compliance Inspection conducted on 10/02/2024 and are subject to changes pending programmatic review. The Emergency Disaster Contact Form was completed. The Empower Self-Evaluation invitation link was sent to the Provider. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Toilet paper dispensers will remain secured to the stall wall *Field Trip Permission Forms will indicate if a child is absent and will not attend the trip Compliance Officer is Heather Bauer.
