Findings: The provider has 2 children who had the lead blood test done at 12 months but are now 3 and 4 years old. Documentation of the lead blood test done on or after 24 months is needed. The provider is in touch with the parents of those children to either obtain the lead blood tests documentation or doctor appointment verification. EB's second lead blood test was done last week and the parent is waiting on the results. MP's mother stated her 12 month lead blood test was attempted at the 15 month well-visit but the doctor's office blew her vein out and could not complete the lead blood testing. The provider is in contact with that parent to determine if and when the lead blood test will occur.
