He also needs documentation of his updated immunization records which will show his 12 month immunizations. Two siblings began care on 3/21/2018 and do not have part II of the health inventory to include the lead screening. The siblings also do not have documentation of immunizations in their file. The siblings have a documented medical appointment on 4/12/2018. The two infants enrolled are 15 months and 16 months old. They will need documentation of the lead blood tests on page 3 of the updated health inventory. The revised health inventory form was given to the provider.|
|Findings: The provider's daughter turned 18 in June 2017. She has not yet submitted the original notarized release to OCC yet. Her daughter is still a resident but is away at college. Please forward the original notarized release to OCC as soon as possible. This form can be completed by the daughter at college; she must have a notary notarize the form though.|

The provider stated the daughter is looking into getting her fingerprinting done closer to college. This should be done as soon as possible as she has been 18 for almost a year.|
|Findings: The provider did not complete any continued training during the 2016-2017 training cycle. She has completed the required basic health and safety training during this training cycle. The provider owes 9 more hours to make up for last year's continued training. She will then owe 12 more hours of continued training by 6/30/2018. Please forward copies of the training certificates to OCC once completed.|
|Findings: The provider stated she is behind on her fire drills and disaster drills. The provider is reminded to start conducting fire drills at least monthly and do disaster drills at least twice per year. The drills must be documented. A new drill form was given to the provider. Notify the licensing specialist as soon as the next drill is conducted.|
