An extension was given to providers in regards to continued training due during COVID. This does not apply to training due prior to the COVID emergency (due by 12/31/2020). Please submit proof of 23 hours of continued training to the licensing specialist once completed. Note: The provider's CPR/first aid continued training expires in 7/2020. Due to the COVID emergency, the provider has not yet updated her first aid/CPR. There is an extension until 12/31/2020 to get this completed. Findings: One of the infants enrolled is missing the health inventory which includes the individual needs information and lead screening. He also needs documentation of his updated immunization records which will show his 12 month immunizations. Two siblings began care on 3/21/2018 and do not have part II of the health inventory to include the lead screening. The siblings also do not have documentation of immunizations in their file. The siblings have a documented medical appointment on 4/12/2018. The two infants enrolled are 15 months and 16 months old. They will need documentation of the lead blood tests on page 3 of the updated health inventory. The revised health inventory form was given to the provider.

Findings: The provider's daughter turned 18 in June 2017. She has not yet submitted the original notarized release to OCC yet. Her daughter is still a resident but is away at college. Please forward the original notarized release to OCC as soon as possible. This form can be completed by the daughter at college; she must have a notary notarize the form though.
