After reviewing Code 3280.124(b)(2) . I understand we received this violation because the work information was not written . This information must be filled in when a parent is not working. I will make sure this is written on the space that request the work information. In the future, I will make sure all sections are filled in on the emergency contact form when the parents fill them out. I am attaching an updated emergency contact form filled out correctly on 7/13/2020 for child#3.

After reviewing 3280.182(1) I understand that I must always keep initial physicals in each child's file . In the future, I will make sure all initial health physicals are in each file . I will also train my staff during the next staff meeting on Friday,July 31,2020. I will send the initial physical I have in child #4 file along with this document.

After reviewing codes 3280.131(a)(4) and 3280.181(1) I understand that I must have initial health records for all children. We will make sure that all staff is aware of this requirement. I will retain all medical records in each child's file at all times. In the future I will attach initial files for child # 1,2,and 3 . Children that will be returning for care will be required to have a current health form based on when Drs will schedule in person physicals.
