The provider couldn't find any admission forms for the an infant, about 7 months old, who the provider called C. Please immediately obtain a completed emergency form for CM. Findings: Upon arrival Licensing Specialist observed a pillow in the pack n play where an infant was laying. Both of the pack n plays had blankets in them with the infants. Once discussed the provider removed the pillow and extra linens. Please be sure that there is no soft bedding in any infant nap sources. Please be sure that infants are sleeping in safe sleep sources only.
