Findings: The play area needs to be mowed or weeded. Resilient surfacing needs to be added to the entire play area. The play equipment needs to be power washed to remove dirt and grime. When this is completed the provider will notify the licensing specialist.

Findings: The butcher block with knives has to be made inaccessible to children and not left on the kitchen counter. The cabinet above the toilet needs a knob replaced and then a lock placed on it to make the toiletries inside this cabinet inaccessible to children. The flooring in the bathroom has to be replaced since the tiles are chipped and present a tripping hazard for children. This is the bathroom that is utilized by the children. When these items have been completed the licensing specialist will be notified.
