Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At the time of the inspection, several staff members were observed leaving their assigned primary groups and moving to other areas of the facility where they were not able to see, hear, direct, or assess the children in their care.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS 1. Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all staff to receive a minimum of three hours of training regarding Supervision. The training must be PQAS approved, in-person, and outside of childcare hours. The legal entity must receive DHS approval of the training content prior to scheduling the training. The correction date for this portion of the plan shall be the date in which training will be completed by all staff.

Description: Infant sleep position

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At the time of the inspection, an infant was observed sleeping in an infant swing. Staff Person #3 indicated that there was no documented medical reason on file from a physician, physician's assistant, or CRNP authorizing the infant to sleep in a position other than that recommended by the American Academy of Pediatrics.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility. TIERED LIS: 1. The operator shall provide a correction date for when the training will be completed.

Description: Disposable diapers/Surfaces cleaned

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, the pedal on the diapering trash can in the Infant Room was not functioning, and the trash can was not hands-free as required. Additionally, cloth coverings were observed on diaper changing pads in the Infant and Toddler rooms, preventing the surfaces from being cleaned and sanitized after each use.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can. Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Description: Pertinent Laws & Regulations/General Health and Safety

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, Staff Person #1, Staff Person #3, and Staff Person #4 were all observed to be caring for children unsupervised at some point during the inspection. Staff Person #1, Staff Person #3, and Staff Person #4 have not completed the required pre-service training prior to caring for children unsupervised, including pediatric first-aid/CPR from a PQAS instructor, mandated reporter training, and CCDBG health and safety training.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre- service trainings are completed, Staff person #1, Staff Person #3, and Staff Person #4 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise Staff person #1, Staff Person #3, and Staff Person #4, they may not work in a child-care position at the facility.

Regulation: 3270.151(a)/3270.151(c)(2)

Description: 12 months prior to service and every 24 months thereafter/Mantoux TB

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, staff files were reviewed. Health assessment on file for Staff Person #3 was not dated. File for Staff Person #4 indicates that they received tuberculosis screening; however, the results were not on file.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, the operator reported that bottles are brought to the operator's home and washed each night. The operator further reported that the facility does not have a commercial dishwasher.

Correction Required: Disposable nursers shall be used for infants unless bottles are provided by the parent or unless a commercial dishwasher is used by the facility.

Description: Bottles labeled

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, several unlabeled bottles were observed in the facility. An infant was observed being fed from an unlabeled bottle, which was not labeled as required.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, staff files were reviewed. Files for Staff Person #1 and Staff Person #4 did not include any verification of child care experience, education and training prior to service at the facility. File for Staff Person #3 did not include any verification of education prior to service at the facility.

Correction Required: A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, staff files were reviewed. The files for Staff Person #1 and Staff Person #4 did not contain any written, nonfamily references. The files for Staff Person #2 and Staff Person #3 each contained only one written, nonfamily reference.

Correction Required: A facility person's record shall include two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, the letter provided to parents at enrollment explaining the facility's emergency plan did not include a method for contacting parents as soon as reasonably possible during an emergency, or a method for notifying parents when the emergency has ended and providing instructions for safe reunification.

Correction Required: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises. The facility shall have an emergency plan that provides for a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children.

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, the letter provided to parents at enrollment explaining the facility's emergency plan did not include accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions.

Correction Required: The facility shall have an emergency plan that provides for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions.

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, staff files were reviewed. Last documented fire safety training certificate on file for Staff Person #5 is dated 11/20/2024, which is over 12 months ago.

Correction Required: Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Description: Health and Safety Training

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, staff files were reviewed. Staff Person #3 has not completed the following required pre- service training within 90 days of their date of hire (see LIS code sheet): pediatric first-aid/CPR from a PQAS instructor, mandated reporter training, and CCDBG health and safety training.

Correction Required: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Until such time as the required training has been completed, Staff Person #3 must be supervised, when interacting with children at a minimum by, an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise Staff Person #3, Staff Person #3 may not work in a child-care position at the facility.

Description: Comply with CPSL/CPSL information

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, staff files were reviewed. The file for Staff Person #1 contained a volunteer child abuse clearance dated 2/3/2026, which is not an acceptable clearance. No PSP clearance was on file for Staff Person #1. During the inspection, Staff Person #1 emailed a volunteer PSP clearance dated 2/3/2026 to the director, which is also not an acceptable clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #1 may not work in a child care position with direct contact and routine interaction with children until all completed clearances are on file at the facility. TIERED LIS: 1. The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person #1 may not work in a child care position with direct contact and routine interaction with children until all completed clearances are on file at the facility. The Legal Entity, Director, and any other facility person(s) responsible for hiring and maintaining staff files must register for and attend the Existing Provider Orientation presented by a Regional Office. The operator shall provide a date for when this training will be completed.

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, several outdoor toys were observed in front of a marked fire exit, obstructing the means of egress.

Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed.

Noncompliance Area: An allocated unannounced monitoring was conducted on 4/30/2026. At that time, last documented fire drill was conducted on 11/2/2025, which is over 60 days ago.

Correction Required: The Director or designated staff person who is responsible for compliance with this chapter ensure fire drills are conducted at least once every 60 days.

A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. Staff person #1 will not work at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. The CPSL was revised on 9/18/2018 to include a requirement that all clearances be updated and on file at least every 60 months. The Child Protective Services Law (CPSL) was revised to include the requirement for the National Sex Offender Registry (NSOR) Clearance effective 9/30/2019. The CPSL also states that anyone hired after 9/30/19 was required to have the NSOR clearance to work in childcare. Current staff must have the NSOR clearance no later 7/1/20. CPSL was revised to include new hires that have resided out of Pennsylvania with in the 5 previous years must obtain State Police and Child abuse clearances from the state that they have resided. Staff person #1 will not work at the facility.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.
