Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre- service trainings is completed, staff person # 1 and # 2 must be supervised, when interacting with children at a minimum by, an (AGS, primary staff person, or family childcare home staff person) who has completed all preservice trainings and has all qualifications to care for children unsupervised. Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 2 may not work in a childcare position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, DHS FBI Fingerprint clearance, National Sex Offender Registry Certificate, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Noncompliance Area: On March 19, 2025, during the renewal inspection, the telephone number of the nearest hospital, police department, fire department, ambulance and poison control center was not posted by the white cordless telephone sitting on the ledge of the toddler childcare space.

Correction Required: The telephone number of the nearest hospital, police department, fire department, ambulance and poison control center shall be posted by each telephone in the facility.

Description: Age appropriate/Large muscle development

Noncompliance Area: On 11/01/24, during the initial inspection, insufficient amounts of large motor toys were observed in the play spaces. Three toys for large muscle development were observed in the outdoor play space for older toddlers and preschool children. There was one large muscle development toy located in the infant and young toddler play space.

Correction Required: Play equipment and materials appropriate to the developmental needs, individual interests and ages of the children shall be provided in sufficient amount and variety to preclude long waits for use. Play equipment and materials shall include toys and equipment for large muscle development, prior to the issuance of a certificate of compliance.

Description: Hazardous toys and equipment

Noncompliance Area: On 11/01/24, during the initial inspection, there was no evidence of the cribs in the center being in compliance with the updated crib United States Consumer Product Safety standards.

Correction Required: Children's toys and equipment, including furniture and rest equipment, described as hazardous by the United States Consumer Product Safety Commission may not be used by children at the facility and may not be on the premises at the facility. At the time of inspection, the operator shall submit to the Department written affirmation on a form provided by the Department stating that the facility is in compliance with this requirement, prior to the issuance of the certificate of compliance.

Description: Small Toys and Objects

Noncompliance Area: On 11/01/24, during the initial inspection, toys in both classrooms were observed to be in the plastic bags that they came in. Cot sheets, and crib mattresses were also observed to be in their original plastic packaging. Small tags were observed to be attached to the chairs and highchairs throughout the center.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths, prior to the issuance of a certificate of compliance.

Description: Disposable diapers

Noncompliance Area: On 11/01/24, during the initial inspection, the hands free waste receptacle by the front diapering table in the infant classroom did not have a plastic liner.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can, prior to the issuance of a certificate of compliance.

Correction Required: The facility shall have an emergency plan that provides for a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises. Noncompliance Area: On 11/01/24, during the initial inspection, first aid kits containing Tylenol and Neosporin were located on top of the cubbies and accessible to children in the main classroom and the back classroom.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children. A first-aid kit must be inaccessible to children, prior to the issuance of a certificate of compliance.

Noncompliance Area: On 11/01/24, during the initial inspection, no tweezers, soap, scissors, or nonporous gloves were found in any of the four first-aid kits located in the center.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves, prior to the issuance of a certificate of compliance.

Description: Handwashing signs

Noncompliance Area: On 11/01/24, during the initial inspection, there were no handwashing signs with the written requirements "Staff and children must wash hands before eating and after toileting" in any of the toilet, diapering, or sink areas at the facility.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility, prior to the issuance of a certificate of compliance.

Description: Lidded waste receptacles

Noncompliance Area: On 11/01/24, during the initial inspection, a lidded waste receptacle was not observed by the classroom sink area of the infant classroom, and the bathroom of the infant classroom.

Correction Required: A toilet area, training chair area, diapering area and sink area shall be equipped with a clean, lidded waste receptacle, prior to the issuance of a certificate of compliance.

Description: Running water - diapering

Noncompliance Area: On 11/01/24, during the initial inspection, the changing table near the window in the infant classroom is not near a source of running water.

Correction Required: A source of running water for handwashing shall be present in infant and toddler diapering areas. If the running water does not flow directly into a drain that is connected to a sewage system, a receptacle shall be provided to contain the water used for washing. The receptacle shall be emptied into an approved sewage system at least once a day, prior to the issuance of a certificate of compliance.

Noncompliance Area: On 11/01/2024, during the initial inspection, proof of purchase for the facility's fire detection system was not provided.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. §  1016(c)). The Director or designated staff person who is responsible for compliance with this chapter shall ensure the requirements under subsection (a) are met, prior to issuance of a certificate of compliance.
