A complete inspection could not be conducted due to the temporary closure of the child care group home. The temporary closure was not notified to the department. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies.

Initial Comments: The following deficiencies were observed at the time of the Initial inspection conducted on 09.03.25, and are subject to changes pending programmatic review. Please submit the written Documentation of Corrections via the LMS portal within 10 days. Two fingerprint clearance cards were reviewed were verified as valid via the DPS website at the time of the inspection. The following was discussed but not limited to: *Ensure the back of the Criminal History Affidavit is attached in the file. *Submit fire extinguisher receipt. *Ensure menu is dated. *Ensure screen time is added to the schedule. *Ensure outlets are covered in the outdoor area. *Ensure gauze rolls are added to the First Aid kit. *Ensure any residents have a resident file.
