| Initial Comments: The following deficiencies were found at the time of the Initial Inspection conducted on 9/24/2025, and are subject to changes pending programmatic review. The Plan of Corrections is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but is not limited to: *Ensure Criminal History Affidavits are complete with the facility name, address, and back side, *Ensure background check results are received before a staff's starting date of employment, *Ensure that a staff's fingerprint clearance card is verified before the staff's starting date of employment, *Ensure staff files are complete with the necessary education documents, *Ensure the refrigerator and freezer have thermometers, *Ensure hazards are inaccessible when using the gym, *Ensure fans are mounted and inaccessible, *Ensure to have the required posting in each activity area, and *Ensure gallon size bags are located in the first aid kit. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. | |||
