Ellsworth Elementary School is the third elementary school within the J.O. Combs Unified School District. We educate nearly 900 Kindergarten through 6th grade children. Located in San Tan Valley, Arizona, we pride ourselves on having the best of the “small town” benefits while, at the same time, looking to the future with the most advanced technology and conveniences of the 21st century. We believe you can have it all! Our school has attracted the best of the best teachers in Arizona. They care about the academic and social growth of our students. Being a neighborhood school, we care about each other and our accomplishments as families do. Parents and teachers work together for the personal success of each of our students, instilling personal qualities such as respect, responsibility, trustworthiness, and caring. We’re not just educating children; we’re raising and educating successful adults, citizens, and leaders of communities.

Initial Comments: The following deficiencies were observed at the Annual Inspection conducted on 4/23/2026, and are subject to change pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided during the inspection. The Emergency Disaster Contact form was emailed to the director to be completed. The DES group size checklist was compliant at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 3 of 3 fingerprint clearance cards were verified as valid via the DPS website during the inspection. The following was discussed, but not limited to: *Screen time is to be documented on the lesson plan. *The use of an Accident, Injury, and Illness log. *Maintenance of playground equipment.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 4/21/2025 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided at the time of the inspection. Please submit a Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The DES Group Size form was completed at the time of the inspection. The fingerprint clearance cards for 6 of 9 staff members were verified to be valid through the DPS website at the time of the inspection. The fingerprint clearance cards for 3 of 9 staff members were verified to be valid following the inspection.

Initial Comments: The following deficiencies were observed at the time of the Compliance inspection conducted on 4/26/24, and are subject to changes pending programmatic review. Compliance Officer #1: Jennifer Forschino A full inspection was conducted at this time. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The following items were discussed but not limited to: Ensure any broken equipment on the outside playground is repaired or removed. Ensure any trash cans with food waste have a lid.
