Initial Comments: There were no deficiencies observed at the time of the Annual Compliance Inspection conducted on 10/1/2025. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: *Discussed the new rule set and the location of the new forms.

Initial Comments: No deficiencies were observed at the time of the Complaint Investigation conducted on 8/28/2025 for case #00138057. This report is subject to changes pending programmatic review. A full inspection was not conducted at this time. Ratios observed were: Preschool Classroom #207 (3-5 year-old children): 2:16 There were 2 staff interviewed during the inspection. The Compliance Officer attempted to speak with the Complainant by phone and email. Upon completion of the investigations for Complaint #00138057, it was determined from observations and interviews that the allegation was unable to be substantiated due to insufficient evidence. During the exit interview, the following items were discussed, but not limited to: *Discussed the new rules set and the new documentation available on the website. *Discussed which staff members need complete files and that all documentation must be kept for at least one year.

Initial Comments: There were no deficiencies observed at the time of the Monitoring Inspection conducted on 12/3/2024. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The DES Contact Group size was in compliance at the time of the inspection. Initial Comments: No deficiencies were observed at the time of the Initial Inspection conducted on 10/9/2024. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *The placement of cones at the edge of the building blocks access to the back of the school. *The placement of cones at the rock water basin on the Southwest part of the playground. *The water drain in the middle of the playing field needs to be filled in with dirt meeting the top of the drain. *The sharp wire edges need to be removed from the tree and stake located on the Southeast corner of the playground.
