A copy of the Notice of Inspection Rights was provided at the time of the inspection. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1. Staff file documentation. 2. Food waste stored indoors and outdoors. 3. Medication. 4. Service classifications. 5. Updating the facility's director via the portal. 6. Posting a refund policy.

Initial Comments: The following deficiencies were observed at the time of the initial inspection conducted on 6/27/2025, and are subject to changes pending programmatic review. 1 of 1 fingerprint clearance card was verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection The following was discussed but, not limited to: 1. Children’s sign-in and out records. 2. Children's Emergency Information and Immunization records and telephone authorization code. 3. Staff attendance records. 4. Accessible drinking water. 5. Monthly fire drills. 6. Classroom rosters. 7. Notification of pesticides. 8. Child abuse and neglect- mandated reporter. 9. Written approval for personal products supplies by the facility. 10. Children with special needs and in the individualized plan. 11. Discipline and guidance procedures. 12. Written accident, evacuation, and emergency plans. 13. Notifying parents and documenting injuries. 14. Illness and infestation log. 15. Medication procedures. 16. Transportation and Field Trip Procedures. 17. Notify the Department of closure for any days. 18. Interview rights of staff and children.
