School-aged children were not adequately supervised. At 4:20pm a child enrolled in the group with kindergarten through second grade children was observed leaving the cafeteria area and walking towards the exit door where parents pick up. A group leader came out and asked the child where she was going. The child said she was leaving. The group leader escorted the child back into the cafeteria. There was a group leader standing at the bathrooms but was unaware the child had left the cafeteria and could not see or hear the child. | |||

Two (2) of the three (3) children were observed walking down the hallway. The third child who was seven years of age was observed going into the restroom. | |||

School-aged children were not adequately supervised.unlicensed program and meet the requirements for administrators as required by GS 110-91(8). During arrival while children were gathering in the cafeteria one (1) kindergarten age child needed to use the restroom. The restroom is located outside of the cafeteria and down the hallway were children can not be seen or heard due to the noise from the children in the cafeteria. A staff member was observed escorting the child to the restroom but returned to the cafeteria before the child. The child was observed rejoining the group a few minutes later. |
